Supported Living Services

Pathway To Choices' Supported Living Service is similar to ILS, but is provided to clients who require ongoing training and support to live in their own home. The duration of the services are provided based on an ongoing assessment of need. Supports are available as often and for as long as is needed, based on the availability of natural, generic, and other supports. The number of training and support hours will vary and can be provided up to 24-hours a day. 🔗
